Your coordinator is an experienced oncology nurse who will guide you through your individual care pathway. They are a caring familiar face who will support your clinical treatment, your emotional and psychosocial needs, and provide reinforcement and clarification of information to assist through your decision-making process. This comprehensive, coordinated service means you will feel supported every step of the way.
Your cancer care coordinator is part of your multidisciplinary team and assists in the planning and coordination of your clinical care. Icon delivers integrated cancer care and can refer you to additional trusted allied health providers and support services should these be required, such as:
Community support services
Dietician
Exercise physiologist
Occupational therapist
Physiotherapist
Psychologist
Social worker
Speech pathologist
Providing personalised support to guide you through diagnosis, treatment and beyond
Facilitating access to allied health and support services
Providing information about your cancer, procedures and treatment
Liaising with hospitals and community services or agencies regarding your ongoing care
Providing patient advocacy services when needed

To make an appointment with an Icon cancer care coordinator, please speak to our friendly administration team at your local centre. A doctor’s referral is not required to access cancer care coordination services and referrals can be made by patients, carers or healthcare professionals.
